A tragic incident at Birmingham Children’s Hospital has led to the untimely death of three-year-old Aarav Chopra, who passed away following what was meant to be a routine medical procedure. The child, who had previously undergone a liver transplant, succumbed to significant bleeding after a trainee doctor inadvertently pierced an artery during a liver biopsy on 22 November 2023.

The Birmingham Women’s and Children’s NHS Trust, which oversees the hospital, has publicly acknowledged its role in the tragedy, offering a heartfelt apology to Aarav’s family. Dr. Satish Rao, the Chief Medical Officer of the trust, expressed his condolences, admitting that the care provided fell short of the expected standards. “We know that the standard of care provided did not meet those expected and for this we are truly sorry and have admitted full liability,” Rao stated. He also assured that changes have been made to improve patient care in light of Aarav’s death.


According to a prevention of future deaths report released by the UK Courts and Tribunals Judiciary, Aarav experienced a cardiac arrest due to severe bleeding that was related to the damage of an intercostal artery during the biopsy. This complication went undetected and untreated at the time, ultimately leading to his tragic demise. The report highlighted a series of systematic failures, noting that there was inadequate planning before the procedure, including a lack of proper communication regarding the risks associated with Aarav’s case.
Aarav had been born with biliary atresia, a condition that obstructs bile flow from the liver, necessitating the transplant he had previously endured. The coroner’s report specified that his cause of death included hypoxic-ischaemic encephalopathy, a condition caused by a lack of oxygen and blood supply to the brain, alongside significant bleeding into the pleural space—which ultimately pressured his heart—due to the biopsy.
Following this distressing loss, Aarav’s mother, Amrita, shared the emotional toll the incident has taken on the family. She expressed their devastation and anger, stating, “We took Aarav to a really good place… and then they’ve basically killed him.” Amrita emphasised her desire for other families not to experience similar heartache, reflecting the profound grief and sense of betrayal the family feels.
Aarav’s father, Kishore, remembered his son as an “adorable little boy” who always radiated happiness, despite the challenges he faced during his illness. “He brought sunshine to our lives and the world is a much darker place without him,” he lamented, acknowledging how their hopes for a new beginning through the transplant were shattered by what they believed would be a straightforward procedure.
The investigation into the circumstances surrounding Aarav’s death revealed significant lapses in communication, particularly concerning consent. It was highlighted that Aarav’s parents were unaware that a trainee would be performing the liver biopsy. Additionally, they later learned that the individual was only a fourth-year trainee, contrary to indications that they were more experienced.
The family’s legal representation, Catherine Lee from Irwin Mitchell, corroborated that the trust had recognised the severity of the failings that contributed to Aarav’s death. She added that it was essential for hospital staff to be consistently supported to maintain the highest standards of care.
The tragic incident highlights significant concerns regarding the safety and protocols of routine procedures in healthcare settings, drawing attention to the paramount importance of communication and thorough preparation in medical environments. The Birmingham and Solihull Coroner’s Court has yet to respond to inquiries for further details regarding this case.
