In 2025, Joel experienced alarming symptoms, initially noticing his balance had begun to deteriorate. Concerns heightened when he fell during a performance, prompting a visit to a physician. After being diagnosed with NPH, a condition characterised by the accumulation of excess cerebrospinal fluid in the brain’s ventricles, he took immediate action and commenced physical therapy to address his troubling symptoms.

During the SiriusXM interview, Joel recounted his apprehension regarding the recommended surgical procedure, which involved drilling a small hole in the skull to implant a shunt to alleviate pressure on the brain. “I said, ‘Nah, I think I’ll pass on that,’” he recalled, highlighting his initial fear. However, after researching the potential benefits of the procedure on his balance and memory, Joel decided to proceed with the surgery, stating, “I don’t want to lose my marbles.”
After undergoing the operation, Joel has reported significant improvements in his memory, balance, and overall ability to perform daily tasks. However, the musician noted that he has had to reduce the frequency of his concert appearances. Medical professionals advised him that the loud environment associated with performing rock music could exacerbate his condition, leading to cumulative damage to his brain.
Reflecting on the advice from his doctors, Joel shared, “They said it was like having a concussion every time I did a show. I could be losing more brain cells, and I realised I needed to make changes.” This cautious approach has led him to focus on recovery, spending time on physical therapy and adhering to medical guidance to prevent further complications.
Despite the challenges, Joel expressed reassurance to his fans, clarifying, “I just want people to know, don’t worry about me. I’m not going anywhere. I’m still here.” His light-hearted demeanour and commitment to recovery have resonated well with his audience, as many have expressed their concern over his health.
The “Vienna” star first disclosed his diagnosis publicly in May 2025, coinciding with the cancellation of several performances. At that time, he explained that NPH had intensified his struggles with hearing, vision, and balance during concerts. In a subsequent interview with PEOPLE, he maintained an optimistic outlook, stating, “I’m simply rolling with the punches.”
Recalling the onset of his condition, Joel remarked that the experience was indeed frightening but emphasised his improving health state. He continued, “I just wanted to let people know, don’t worry about me being deathly ill or anything.” This reassurance has been welcomed by his fans, demonstrating their support for his journey to recovery.
Recently, Joel surprised attendees at an amphitheatre in Wellington, Florida, where he made an unexpected appearance during a Billy Joel cover band’s performance. Fans were thrilled to see him on stage, and he humourously acknowledged his impromptu performance, quipping, “I wasn’t planning on working tonight,” before launching into renditions of “We Didn’t Start the Fire” and “Big Shot.”
As Joel continues to manage his health, he remains committed to his passion for music while ensuring he takes the necessary steps towards recovery. With the combination of medical assistance, physical therapy, and his enduring spirit, Joel is determined to make the most of his time on stage while prioritising his well-being.
