This decision was motivated by her Type 1 diabetes, which increases the risks of experiencing the aftereffects of anaesthesia.

Patty Peasley, who struggles with moderate to severe hearing loss, sought out the implant to improve her quality of life. Cochlear implants are intricate devices that provide a sense of sound by bypassing damaged sections of the ear and directly stimulating the auditory nerve, according to the National Institute on Deafness and Other Communication Disorders (NIDCD).

Typically, cochlear implant surgeries are performed under general anaesthesia, which induces a sleep-like state in patients. However, Peasley chose a local anaesthetic instead, administered solely around the ear. She was determined to avoid the drowsiness and confusion that can follow general anaesthesia, a concern heightened by her management of diabetes. “I’m on an insulin pump, and it helps me not to be foggy with the anaesthetic,” Peasley explained, underlining her desire for comfort during the procedure.

The surgery was conducted by Dr. Varun Varadarajan, a specialist in neurotology and otology, who has previously pioneered similar procedures in the Sacramento area. Peasley’s surgery involved a small incision behind her ear and the creation of an opening in the mastoid bone to position the implant’s electrodes effectively within the cochlea.
In a unique twist, while undergoing the surgery, Peasley chose to listen to music. Dr. Varadarajan revealed that her preference was for classical piano, leading him to select works by Chopin for the procedure. Peasley described her experience as reminiscent of a dental visit, stating, “There was some drilling going on, but that was not an issue for me.” She highlighted the role of the medical staff, particularly a nurse who did a “beautiful” job of guiding her through some of the more challenging sensations experienced during the operation.
Awake cochlear implantation is an innovative option that can aid patients who face heightened risks from general anaesthesia, such as older adults or those with pre-existing health conditions. Experts from UC Davis Health confirmed that for suitable candidates, this method of surgery can lead to quicker recovery times and fewer complications like post-surgery nausea and grogginess.
The procedure itself has evolved significantly, now possible in an outpatient format that typically lasts between one and three hours. As Dr. Varadarajan noted, most patients can expect to return home on the same day and begin hearing shortly thereafter.
Reflecting on her journey, Peasley shared the importance of her decision to pursue the cochlear implant, describing it as a turning point in reconnecting with life. “I realised for quite some time that I was missing an important part of my life,” she remarked, adding that the implant “really is changing my life,” and she looks forward to what better hearing will bring.
