The Transportation Safety Board of Canada (TSB) released its comprehensive final report on the tragic implosion of the Titan submersible on June 17, 2023. The document highlights critical structural flaws in the Titan’s carbon fibre hull and lapses in regulatory oversight as significant factors contributing to the disaster, which claimed the lives of all five passengers aboard during an expedition to the Titanic wreck site.

The report, which spans 136 pages, lays responsibility at the feet of OceanGate, the US-based company responsible for the Titan. It argues that OceanGate was influenced by a culture of “groupthink” and “confirmation bias,” which led to a dangerous underestimation of the risks associated with the untested vessel. Notably, the TSB revealed that the fundamental properties of the Titan’s hull material had not been adequately validated before its deployment.


Investigators examined remnants of the materials used in the Titan’s construction and discovered alarming structural defects. Although OceanGate conducted tests simulating a single deep dive similar to those required to reach the Titanic’s depths, they failed to assess how repeated descents would weaken the submersible. The report suggests this oversight left OceanGate unsure about the durability of the Titan’s pressure hull when subjected to repeated extreme conditions.
The TSB report also highlighted serious deficiencies in international oversight for submersible craft like the Titan. While the agency acknowledged awareness of OceanGate’s operations in Canada, it lacked any regulatory authority over the company, which limited its ability to monitor the safety risks involved in the Titan’s missions. The report explicitly noted, “No one was responsible for connecting the dots,” painting a picture of insufficient checks and balances within the industry.
TSB Chair Yoan Marier emphasized the urgent need for more stringent regulatory measures in the marine sector, asserting that “lives are at risk when safety gaps are left unaddressed.” Marier has been advocating for increased oversight for several years, underscoring the heightened risks posed by unregulated exploration.
The report detailed potential incidents where the Titan might have endured damage, including a collision with the Titanic’s bow in 2022 and a loud noise as it surfaced from a subsequent dive. Furthermore, the submersible had been exposed to harsh ocean conditions for nearly a year leading up to its final voyage, potentially exacerbating existing weaknesses in its structure. The document warned that repeated stress on the vessel could lead to significant damage accumulations over time.
In a related investigation, the U.S. Coast Guard’s final report concluded that had OceanGate’s co-founder Stockton Rush survived, he may have faced manslaughter charges. The report pointed out that there was no precedent for submersibles made of carbon fibre being piloted to such extreme ocean depths and acknowledged the inherent risks outlined by OceanGate, both internally and publicly.
The Titan was a 22-foot-long submersible designed to reach the Titanic wreckage resting at the bottom of the Atlantic Ocean. Tragically, nearly two hours into its journey, all communications were lost, prompting a wide-ranging international search effort. On board were prominent figures including Hamish Harding, a British adventurer; Shahzada Dawood, a notable businessman from Pakistan, alongside his son Suleman; Paul-Henri Nargeolet, an expert on the Titanic site; and Stockton Rush himself.
After four days of exhaustive efforts to locate the Titan, investigators found the wreckage approximately 400 miles off the coast of Newfoundland. It was concluded that the passengers likely perished instantly as the vessel imploded near the famous wreck.
The TSB’s report serves as a somber reminder of the risks associated with deep-sea exploration and raises critical questions about the accountability of commercial ventures in this increasingly popular field. Safety measures must be prioritised to prevent future tragedies, as exemplified by the Titan disaster, to ensure the well-being of those who seek adventure beneath the sea’s surface.
