Bethenny Frankel, the former star of “The Real Housewives of New York City,” recently shared her evolving stance on cosmetic procedures during the launch celebration of the 2026 Sports Illustrated Swimsuit Issue. Speaking exclusively to PEOPLE, Frankel addressed her experiences with beauty treatments, shedding light on her choices and preferences in the ever-changing landscape of cosmetic enhancements.

At 55 years old, Frankel candidly admitted that she has experimented with fillers in the past. However, she has since decided to steer clear of them. “I didn’t do it a lot, but I did it — and then I heard it never leaves your face,” she explained at the launch event held at the Hard Rock Hotel in New York City. She expressed a desire to avoid any beauty treatment that could become a permanent fixture on her face, pointing to her firm decision not to use fillers again.
Despite her aversion to fillers, Frankel does still engage in some cosmetic procedures. She confirmed that she uses Botox, stating, “I Botox, yes. Not filler.” This distinction highlights her selective approach to beauty treatments, which she feels comfortable incorporating into her routine without going overboard.

Reflecting on her past experiences, Frankel recounted one of the most unconventional treatments she has tried: a vampire facelift. “The wildest, I did that vampire thing where they prick your face everywhere with blood. It all scabbed up,” she recalled. The vampire facelift, which involves injecting platelet-rich plasma, is said to revitalise the skin without surgical intervention. While this procedure can provide a refreshed appearance, it is associated with potential side effects like bruising and swelling.
Frankel made it clear that she approaches beauty treatments with caution, stating, “I’m not big on gimmicks.” Her decision to avoid the latest trends in cosmetic procedures comes from a desire for informed choices rather than succumbing to societal pressures. “I don’t know enough,” she emphasised when discussing her hesitance to try new treatments.
She went on to mention a particular procedure she would never consider: thread lifts. This non-surgical method involves inserting threads under the skin to achieve a lifted look while promoting collagen production. “I heard not to do the string thing,” Frankel commented, underscoring her wariness regarding certain beauty enhancements.
Amid her reflections on cosmetic procedures, Frankel expressed enthusiasm about her debut in the Sports Illustrated Swimsuit Issue. The photoshoot, set against the stunning landscapes of Baja California Sur, Mexico, has been a highlight of her year. She spoke positively about the experience, saying, “I thought they were great. I loved doing it. I loved getting it.”
Her boyfriend, Shane Campbell, has been supportive of her venture into the swimsuit modelling world. When asked about his reaction to the published photos, she shared, “He loved it. He thought it was beautiful. He was excited.” Their relationship, which gained public attention earlier this month, appears to be flourishing as they navigate their new romance.
Frankel and Campbell made their first public appearance earlier this year at the Global Champions Arabians Tour in Miami Beach. Their chemistry has been evident, especially as they openly share their affections on social media.
As she reflects on her beauty journey and the choices she makes, Bethenny Frankel continues to prioritize authenticity and well-informed decisions over fleeting fads. Her experience serves as a reminder that self-acceptance and confidence can be bolstered by genuine choices in the realm of beauty and wellness.
