In a tragic incident that has shaken the local community, Matthew Miller, a 48-year-old man from Ohio, died following a routine dental procedure that took a disastrous turn. Miller underwent surgery at the Visage Surgical Institute in Medina on 11 August 2023, and his widow, Brandy Miller, has since revealed the painful details of the events that unfolded.

Brandy Miller stated that she and their youngest son waited anxiously outside the surgical office while Dr Faisal Quereshy performed the extraction of three teeth on her husband. Concern quickly grew when Brandy inquired about Matthew’s condition, only to be met with distressing news. She recounted her conversation with Dr Quereshy, recalling, “He said, ‘Not good,’” before explaining that her husband had stopped breathing and that paramedics had been called to attend to the situation.

As emergency personnel worked on her husband, Brandy described the agonising moment when she held her son’s hand and urged him to pray. Despite their hopes, Matthew was subsequently taken to a local hospital, where he remained for four days before being pronounced dead.
An investigation by the Medina County Medical Examiner’s Office revealed that the cause of death was attributed to complications from sedation during the procedure. The Ohio State Dental Board is currently looking into the incident, with findings indicating that Dr Quereshy may have neglected to conduct a thorough medical evaluation prior to sedating Matthew. Furthermore, it was suggested that he failed to monitor his patient adequately during the surgery.
In light of these developments, a filmed deposition from Dr Quereshy emerged, during which he was asked if he would approach the situation differently in hindsight. His response was surprising; he stated, “No,” asserting that nothing in Matthew’s evaluation indicated an increased risk for anaesthesia complications.
Brandy expressed her anguish and incredulity at Dr Quereshy’s stance. She is struggling with the idea of forgiveness, particularly after witnessing his deposition. “As a Christian, I’d want to say, ‘I forgive you.’ But after seeing his deposition where he said, ‘I wouldn’t change anything,’ I’m not there yet,” she told local news outlet Cleveland 19.
In January, the Ohio State Dental Board recommended a suspension of Dr Quereshy’s dental licence for a period ranging from six months to one year. The hearing examiner concluded that while the incident represented an isolated failure in judgement, a complete revocation of the dentist’s license was not warranted, as there was no evidence of systemic malpractice.
Brandy Miller has vowed to seek justice for her husband and prevent similar tragedies in the future. She commented, “Whatever we need to do to keep this from happening, I don’t want anybody to feel what I felt. I don’t want anybody to feel what my kids felt, losing their dad at such a young age.”
Additionally, Brandy has filed a lawsuit against Dr Quereshy, seeking $25,000 in damages, as recorded in public records from the Cuyahoga County Court. The case has drawn attention to concerns regarding patient safety and the responsibilities of dental practitioners.
In the wake of this tragedy, the local community has rallied around the Miller family, offering support in their time of immense grief. Many are now raising awareness about the importance of thorough evaluations and attentive monitoring during medical procedures, especially those involving sedation.
This incident has ignited conversations about the protocols surrounding dental anaesthesia and the importance of safeguarding patients during such procedures. As the investigation proceeds, the Miller family, and the wider community, seek answers and accountability for the profound loss they have suffered. The Ohio State Dental Board did not respond to requests for further comment regarding the ongoing situation.
