According to flight tracking data, ITA Airways flight 621 departed from runway 24L for Rome at around 4:11 p.m. local time, while American Airlines flight 4 took off towards New York City from runway 25R soon after. The ITA Airbus A330-900 immediately banked left after takeoff, crossing in front of the American Airlines flight path.

An air traffic controller alerted the American pilots about the approaching ITA flight, instructing them to halt their altitude climb. The situation was captured in a radio recording obtained by the YouTube channel VASAviation, where the air traffic controller directed the American pilots to the traffic ahead and to their left.

After the American flight changed direction and the ITA flight continued its path, air traffic controllers brought up the incident with the ITA pilots. The pilot offered a brief apology, acknowledging the error in turning south instead of following the designated runway heading.

Witnesses, including YouTuber Kevin Ray who was livestreaming LAX take-offs and landings, noted the abnormal left turn made by the ITA aircraft. Such a maneuver typically requires approval from air traffic control, indicating a potential breach of protocol.
