A 35-year-old man from India recently went through a harrowing experience with blurry vision and eye irritation lasting for a troubling eight months. When he eventually decided to seek medical attention, doctors made a shocking discovery – a worm was found in his eye. This case of ocular gnathostomiasis, a rare parasitic condition, was documented in a case study by The New England Journal of Medicine on Saturday, August 9.

The man had been in distress with blurry vision and eye irritation for an extended period before his visit to the doctor, where the revelation of the eye-dwelling worm emerged. Ocular gnathostomiasis is a condition caused by ingesting raw or undercooked fish, birds, reptiles, amphibians, or mammals that carry the larvae. In this instance, the patient from rural India had consumed infected chicken and fish prior to the diagnosis.

Upon examination, it was noted that the man’s eyes appeared bloodshot, his pupil was fixed and dilated, and his left eye’s vision was limited to 20/80. Furthermore, he exhibited symptoms of panuveitis, a condition involving inflammation spanning from the front to the back of the eye, as elucidated by the Cleveland Clinic. A closer look revealed a worm sluggishly moving in the posterior segment of the eye.
In a bid to tackle the issue head-on, doctors conducted a pars plana vitrectomy procedure to extract the worm from the eye. Subsequent to the procedure, the patient was prescribed oral and ocular glucocorticoids due to the severity of the case. After eight weeks, his symptoms had notably subsided, yet his vision in the left eye remained at 20/40 due to a cataract that developed as a consequence.
Instances of ocular gnathostomiasis are uncommon, particularly in the United States. The condition is predominantly observed in countries where the consumption of raw fish-based dishes, such as sushi, is prevalent. The Centers for Disease Control and Prevention note that countries like Japan, Thailand, Vietnam, and Mexico are more prone to reporting cases of this parasitic affliction.
Overall, this incident sheds light on the importance of seeking timely medical attention when faced with persistent and concerning symptoms. The rapid diagnosis and intervention in this case were crucial in alleviating the patient’s condition and averting potential complications. Awareness about the risks associated with certain dietary practices can also play a pivotal role in preventing such rare yet impactful health issues.
