A heart-wrenching incident has left a family shattered and sparked outrage after a 14-year-old girl lost her life due to being left unattended on a psychiatric ward for a mere 15 minutes. The young girl, Ruth Szymankiewicz, was under care at the now-closed Huntercombe Hospital in the UK, where she was being treated for an eating disorder. Ruth, who also reportedly self-harmed, was supposed to receive round-the-clock supervision at the facility.

According to reports from reputable sources such as the BBC, Sky News, and The Guardian, Ruth was discovered critically injured in her room on February 12, 2022, at Huntercombe Hospital near Maidenhead. Tragically, she succumbed to her injuries two days later at John Radcliffe Hospital in Oxford. The hospital, operated by Active Care Group, has been permanently shut down following the devastating incident.
During the inquest at Buckinghamshire Coroner’s Court, it was determined that Ruth, hailing from Salisbury, was unlawfully killed after being left alone for a brief period at the hospital. Despite requiring constant monitoring, Ruth was found unaccompanied for around 15 minutes, leading to the tragic outcome. She had been admitted to the hospital in October 2021 for treatment related to her eating disorder.
Ruth was placed in the Thames Ward, a psychiatric intensive care unit within the hospital. CCTV footage shown during the inquiry illustrated Ruth’s final moments in the presence of her support worker, identified as Ebo Achempong. Shockingly, Achempong was hired through an agency under false pretenses and repeatedly left Ruth unattended, contrary to the required supervision. Following Ruth’s passing, Achempong reportedly returned to Ghana.
On the fateful evening when Ruth sustained her fatal injuries, CCTV footage displayed her returning to her room around 8 p.m. after her support worker departed the TV room. The inquest unveiled that the unit where Ruth was housed suffered from chronic understaffing, with 50% of the personnel being agency workers on the day of the incident. Moreover, Ruth was able to access harmful online content without intervention.
The jury scrutinizing the circumstances leading to Ruth’s demise concluded that the care provided to her was inadequate and failed to support her recovery. Among the contributing factors highlighted were staff inadequately trained to handle such situations and restrictive visiting policies that limited family interaction. Prior inspections by the Care Quality Commission in 2021 had already raised concerns about the hospital’s operational standards.
The Department of Health and Social Care expressed profound condolences to Ruth’s family and acknowledged the substandard care delivered at Huntercombe Hospital. The department emphasized the necessity for improved mental health services to prevent similar tragedies and affirmed their commitment to investing in enhanced care facilities to facilitate timely and effective treatment.
In a heartfelt statement outside the coroner’s court, Ruth’s parents, Kate and Mark Szymankiewicz, voiced their anguish over the systemic failures that led to their daughter’s untimely death. Mark lamented the ordeal of seeking help for Ruth, only to witness the system that was supposed to safeguard her instead inflict harm. Kate poignantly described the irreplaceable void left by Ruth’s absence within their family.
The Active Care Group expressed deep regret over the tragic incident and extended sincere condolences to Ruth’s loved ones and those impacted by the loss. They distanced themselves from the hiring agency that breached their contract and played a role in the events leading up to Ruth’s demise. The company emphasized their commitment to upholding stringent safety standards and continual improvement in service quality.
In the wake of this devastating incident, awareness about mental health support and care for vulnerable individuals has been reignited. The tragic loss of Ruth Szymankiewicz serves as a stark reminder of the critical need for vigilant supervision and comprehensive care within psychiatric facilities. May her memory inspire positive change in mental health services, ensuring the protection and well-being of all individuals under their care.
