In a terrifying mix-up at a pharmacy, a 9-year-old boy was almost given a highly addictive opioid instead of his usual ADHD medication. Sarah Paquin, the mother of the boy named Declan, recounted the harrowing experience to PEOPLE, expressing gratitude that the potentially devastating mistake was caught in time.

The incident occurred when Sarah’s husband, David, went to give Declan his usual dextroamphetamine medication for ADHD, which they had picked up from Shoppers Drug Mart pharmacy in Comox, B.C., on May 28. As part of their daily routine, David noticed that the medication looked different, leading him to realise that they had been given the wrong prescription – one meant for another customer, containing hydromorphone, a potent opioid.

Dextroamphetamine is a commonly prescribed stimulant for ADHD, while hydromorphone is a potent opioid with a potency significantly higher than morphine, capable of causing life-threatening respiratory depression if misused. Loblaw Companies Ltd., the parent company of Shoppers Drug Mart, acknowledged the error as a case of human mistake, expressing regret for the incident and ensuring steps would be taken to prevent a recurrence.
Sarah shared that the pills given to Declan were a 5 mg dose of the powerful opioid and emphasised the potential catastrophic consequences if her son had taken them. Following the incident, Sarah’s family lodged a formal complaint with the College of Pharmacists of British Columbia to advocate for stricter protocols in handling potent medications, especially concerning children.
Although Declan’s correct medication was eventually located in the pharmacy’s outgoing box, Sarah voiced that the trust in the pharmacy had been shattered, leading the family to seek alternative options for their prescriptions. She urged vigilance among consumers to thoroughly check their medications before leaving the pharmacy, underlining the critical importance of preventing such errors.
The pharmacy’s acknowledgment of the mistake and commitment to reinforcing safety measures is crucial in preventing future errors that could have severe consequences. This incident serves as a sobering reminder of the imperative role pharmacies play in ensuring the accurate dispensing of medications and safeguarding patient health.
As the healthcare system strives to uphold standards of accuracy and patient safety, instances like this underscore the significance of stringent processes and enhanced vigilance at every stage of medication dispensing. The swift response and transparency exhibited by the pharmacy in addressing the error are essential in rebuilding trust with affected families and the wider community.
The resilience and vigilance demonstrated by Declan’s family in addressing the error serve as a poignant reminder of the importance of advocating for robust safeguards in the healthcare system. The episode underscores the need for continuous diligence in medication handling to avert potentially dire outcomes and reinforce patient trust in the healthcare providers tasked with their well-being.
Ultimately, the incident shines a light on the critical need for ongoing diligence and stringent oversight in the pharmaceutical sector to prevent such errors and uphold the sanctity of patient care. With heightened awareness and proactive measures, the healthcare community can work towards mitigating risks and fostering a culture of safety and accountability in medication dispensing.
