On 6 February 2025, a tragic incident involving Bering Air highlighted significant safety concerns in aviation operations within Alaska. A Textron Aviation 208B aircraft, designated as Flight 445, was approaching Nome Airport when it crashed onto pack ice, resulting in the loss of all ten individuals on board, including the pilot. An investigation conducted by the National Transportation Safety Board (NTSB) has raised alarm over the circumstances leading to this catastrophic accident.

In its findings, the NTSB reported that the crash was not due to a singular error, but rather a series of avoidable failures that critically compromised safety margins. NTSB Chairwoman Jennifer Homendy remarked, “The tragedy of Flight 445 was not the result of a single failure, but a series of preventable breakdowns that eroded critical safety margins.” The Chairwoman emphasised the vital role aviation plays in Alaskan communities and advocated for the implementation of new safety measures to prevent such heartbreak from occurring in the future.

Reports indicate that the aircraft was overloaded by nearly 1,000 pounds at the time of the flight, a dangerous circumstance given that it was already facing known icing conditions. The excessive weight significantly affected the aircraft’s stall margin, which is the threshold at which a plane can no longer maintain flight. According to the NTSB, the conditions encountered during the flight included severe icing, with the aircraft experiencing supercooled large droplets shortly after departure.
As the plane neared its destination, it suffered a stall and subsequently lost control. The NTSB’s investigation revealed that the pilot, identified as Chad Antill, did not maintain the necessary airspeed during this critical phase of flight. The report suggested that Antill may have had the autopilot engaged to alleviate his workload while managing the ice protection system, which diverted his attention away from critical flight parameters.
Moreover, the investigation found that Nome Airport had temporarily closed its runways for de-icing, further complicating the situation for the pilot as he prepared for landing. The NTSB concluded that the demands of operating under these conditions severely impaired Antill’s situational awareness, leading to a dangerous decay in airspeed. This combination of factors culminated in an aerodynamic stall from which the pilot could not recover.
The NTSB also pointed to systemic issues within Bering Air’s operational practices. The agency noted a troubling history of repeated overweight flights, branding these occurrences as unsafe and risky. In their final report, they underscored the role of inadequate oversight by the Federal Aviation Administration (FAA) in failing to identify operational complexities and rapid growth at Bering Air as contributing risk factors.
In response to these findings, the NTSB has proposed several safety recommendations aimed at enhancing pilot training specifically focused on preventing and recovering from flight control losses. They have urged both charter and air taxi operators to adopt these measures to bolster safety practices and protect aviation stakeholders.
On 31 July, the FAA issued a statement assuring that the agency takes NTSB recommendations seriously, pledging to conduct a thorough review. While the FAA’s commitment to addressing the issues raised has been acknowledged, Bering Air has yet to respond to inquiries regarding the incident and the recommendations set forth by the NTSB.
The incident has sparked conversations about aviation safety, particularly in remote areas reliant on air transport amidst challenging weather conditions. As agencies analyse and respond to the NTSB’s recommendations, one can only hope that changes will be implemented swiftly to prevent future tragedies similar to Flight 445.
