A recent case study has revealed promising yet extraordinary results involving a woman in her 80s suffering from advanced Alzheimer’s disease. The report, published on May 27 in the journal *Frontiers in Neuroscience*, highlights the temporary improvements in her cognitive function following participation in a psilocybin trial in Brazil.

The unnamed Japanese American woman had experienced a significant decline over the last decade, with symptoms worsening over the past five years. Alzheimer’s disease, which is the most prevalent form of dementia, results in a compromised ability to remember, think, or make decisions, severely impacting daily activities. By the time of her participation in the trial, the woman had become severely cognitively impaired and required assistance with all aspects of her life, including basic communication and mobility.
Psilocybin, the naturally occurring psychedelic compound found in various species of mushrooms, was administered in this trial. Prior to her enrolment, the woman’s legal guardians gave their consent for her participation. Following the administration of a 5-gram dose of psilocybin, researchers observed her entering a prolonged sleep-like state, accompanied by profuse sweating. Remarkably, approximately 19 hours later, she began to engage in extensive autobiographical conversation that lasted several hours—an extraordinary development given her previous condition.

In subsequent days, the woman displayed a range of notable improvements. Reports indicate that she regained urinary continence, increased her walking independence, and demonstrated heightened alertness. Furthermore, she began recognising family members and was able to dress herself—milestones that many thought had been lost to her condition.
A month later, the woman received a second dose of 3 grams of psilocybin. This time, researchers noted that her speech capabilities had further increased. She was described as being more expressive and agile in her movements, while maintaining her regained urinary continence. Researchers involved in the study suggest that these results imply psilocybin could potentially reawaken brain functions in individuals with late-stage Alzheimer’s, though they emphasise that further research is essential before drawing broader conclusions.
Dustin Hines, an associate professor of psychology at the University of Nevada, Las Vegas, expressed his astonishment at the findings, stating that the temporary gains observed in a patient who had faced years of severe impairment are indeed extraordinary. However, he cautioned that this is a single case report and should not be interpreted as evidence of a reversal in Alzheimer’s progression. Hines, who was not involved in the study, stressed that the observations—while remarkable—are temporary, highlighting the need for cautious interpretation.
Despite the hope this case may bring, medical professionals remain vigilant. They point out that psilocybin treatments currently lack broader scientific backing, and some individuals might experience adverse effects. The Food and Drug Administration (FDA) has not yet approved psilocybin for medical purposes, which raises concerns about self-administration or use in individuals with dementia without professional supervision.
Given the real potential for harmful side effects and the unapproved status of psilocybin, Hines advised against individuals attempting psilocybin treatments independently—whether for themselves or for loved ones suffering from dementia. The study, while groundbreaking in its findings, serves as a reminder of the complexities involved in treating neurodegenerative diseases.
