The World Health Organization (WHO) has declared the ongoing Ebola outbreak in the Democratic Republic of Congo (DRC) a public health emergency of international concern. This announcement follows alarming findings from a study published on May 18, which suggests that the scale of the outbreak is significantly larger than previously reported, potentially involving over 1,000 undetected cases.

The study, carried out in collaboration with the MRC Centre for Global Infectious Disease Analysis, indicates that the current Ebola outbreak, caused by the Bundibugyo virus, may have resulted in “substantial” under-detection. As of May 19, health officials reported at least 131 deaths and 513 suspected cases within the DRC, in addition to one death in neighbouring Uganda linked to the outbreak. Dr. Anne Ancia, a WHO representative, emphasised the need for heightened vigilance, stating that investigations reveal the virus may have spread to other provinces and across borders.
In light of these troubling developments, the researcher Dr. Anne Cori, who contributed to the study, highlighted the urgency of enhanced surveillance and coordinated response efforts in the region. “Our analyses suggest that the true number of cases may be substantially higher than those confirmed to date,” Dr. Cori remarked. She noted that the uncertainty of the current estimates underscores the necessity for detailed information collection on cases, which would aid in developing a more effective response.

The situation is further complicated by the fact that symptoms of Ebola can take between two to 21 days to manifest after exposure. Initial signs include fever, muscle aches, and weakness, which can subsequently lead to more severe symptoms such as diarrhoea and vomiting. Unfortunately, there are currently no approved treatments or vaccines for this strain of Ebola, making prevention and rapid response all the more critical.
Interestingly, fears have arisen that the outbreak may have begun several weeks prior to its detection on April 24. In response to the rapid escalation, WHO Director-General Dr. Tedros Adhanom Ghebreyesus convened an emergency meeting in Geneva on May 19. Addressing the severity of the situation, he remarked, “I did not do this lightly. I am deeply concerned about the scale and speed of the epidemic.”
Dr. Ghebreyesus cited multiple factors that heighten the risk of further transmission, including the presence of over 500 suspected cases and a rising death toll among healthcare workers, highlighting the challenges posed by healthcare-associated transmissions. Additionally, the confirmed existence of cases in urban settings such as Kampala and Goma raises significant alarm, given the high population density in these areas.
In response to the outbreak, several countries in the region have tightened border checks. Rwanda has opted to close its borders with the DRC, while Uganda has advised its citizens to avoid physical contact, advising against handshakes and hugs. These measures underscore the urgency and seriousness with which surrounding nations are treating the potential for further spread.
The Centre for Disease Control and Prevention (CDC) has also expressed concern, providing support for response efforts following notification of the outbreak by the Ugandan government on May 15. In a related incident, an American citizen who tested positive for the Ebola virus while in Central Africa is being transferred to Germany, a country experienced in dealing with such cases. Currently, there are no reported instances of the virus in the United States.
As the DRC grapples with the growing outbreak of Ebola, the recent study serves as a reminder of the potential scale of the crisis and the pressing need for effective intervention strategies across the region. The WHO’s declaration highlights the global significance of the situation and raises important questions about the potential for international spread. With health officials working to scale up surveillance, contact tracing, and laboratory testing, efforts are underway to tackle the crisis head-on in what is a crucial moment for public health.
