According to reports from the Boulder County Sheriff’s Office, Takashi was observed descending rapidly from a few hundred feet above the ground in a spin, indicating that his parachute had not fully opened. Witnesses at the scene became alarmed when they saw the parachutist heading towards the ground with his parachute still unopened. Emergency services were immediately contacted, but sadly, Takashi was pronounced dead upon their arrival.

Takashi’s son, Shizuka Koyama, expressed deep sorrow over the loss of his father, describing him as a “remarkable man” who lived life to the fullest. In honour of his late father’s final wishes, Shizuka has set up a GoFundMe page to raise funds for Takashi’s cremation and burial in both the United States and Japan. The touching tribute aims to celebrate the life of a man who held both countries close to his heart.
As the community mourns the loss of Takashi, questions have arisen regarding the safety measures in place during the skydiving experience. Reports indicate that Takashi was flying with Mile-Hi Skydiving at the time of the incident. Alarmingly, this marks the seventh fatality in seven years associated with the skydiving company, raising concerns about the company’s safety protocols.
In response to the tragic incident, Mile-Hi Skydiving has come under scrutiny for the series of fatalities linked to their operations over the years. With previous incidents in 2018, 2019, 2021, 2024, and now in 2025, questions have been raised about the adherence to safety standards within the organisation. The frequency of these tragedies has prompted calls for a thorough investigation into the company’s practices to prevent further loss of life.
The untimely death of Takashi Koyama serves as a stark reminder of the risks associated with extreme sports like skydiving. Beyond the thrill of the experience, there are inherent dangers that participants and operators must be vigilant about to ensure the safety of all involved. As investigations into the incident continue, the community remains united in mourning the loss of a beloved father, son, and skydiving enthusiast.
The outpouring of support for Takashi’s family and the efforts to fulfil his final wishes underscore the impact of this tragedy on those left behind. While the investigation unfolds and questions linger, the memory of Takashi Koyama will endure as a testament to a life lived with passion and determination. The call for transparency and accountability in the wake of such incidents echoes through the community, urging for greater safeguards in high-risk activities to prevent further loss of life.
