Cause of Death Revealed for Woman, 57, Who Refused Medical Care for Nearly 4 Decades

In a tragic case that has shocked the community, the cause of death for a woman who refused medical care for nearly four decades has been revealed following an inquest in Tasmania, Australia. Patricia Iliev, 57, passed away at her home in the town of Sheffield in March 2021, and it was determined by coroner Olivia McTaggart in a report dated August 5 that she died from starvation. The coroner expressed that medical intervention could have potentially saved her life, even in the later stages of her decline.

During the inquest held at the Coroners Court in Hobart, it was disclosed that Iliev weighed a mere 42 lbs. at the time of her death after being bedridden on her couch for months and ceasing to eat during that period. The autopsy conducted by Dr. Donald Ritchey highlighted starvation as the primary cause of death, noting that Iliev had significantly below the normal body mass index and lacked essential body fat stores. Additionally, a gastrointestinal stromal tumour of the stomach and advanced lung disease, attributed to smoking, were cited as contributing factors to her passing.

The Mayo Clinic defines a gastrointestinal stromal tumor (GIST) as a type of cancer originating in the digestive system, commonly found in the stomach and small intestine. The report also stated that the tumor likely suppressed Iliev’s appetite. Furthermore, her battle with emphysema, a chronic lung condition, put her at risk of a wasting syndrome, leading to severe weight loss. Multiple hearings were conducted in Tasmanian cities regarding Iliev’s death, prior to the findings being issued in August 2025.

The inquest explored the role of Iliev’s partner, Philip Adams, in her care and potential contributions to her death. Adams had been looking after Iliev since 2020, as she had experienced multiple debilitating back injuries, confining her to the couch. Despite considerations during the lead-up to her passing, Adams ultimately respected Iliev’s wishes to forgo medical treatment. The coroner determined that while Adams could have done more, he did not intentionally cause her death, displaying affection and providing care upon her requests.

Coroner Olivia McTaggart emphasised that medical intervention could have made a significant difference in Iliev’s outcome and potentially saved her life. She acknowledged the complexities surrounding Iliev’s decisions and Adams’ actions, suggesting that mental illness may have influenced Iliev’s choices.
